1. Submit a screenshot of the Maya app showing your profile. The image must contain your name.

2. Submit a screenshot of the Maya app showing a transaction you made to the platform. The image must contain the following:

• Your name

• Date and amount of the transaction

3. Submit an image of a Maya account statement showing a transaction you made to the platform. The image must contain the following:

• Your name

• Date and amount of the transaction
